Role Overview

Provide expert clinical review using UpToDate Lexidrug or Lexicomp to evaluate medication interactions, dosing, contraindications, and patient-specific safety considerations. This role focuses on confirming that conclusions from drug references hold when applied to a patient''s full medication list and clinical data, catching missed or outdated guidance, and ensuring appropriate follow-through into the chart and care-team communication.

Key Responsibilities
  • Perform complete medication-list interaction checks, assessing severity ratings and interaction mechanisms across the entire active medication list rather than isolated drug pairs.
  • Evaluate contraindications, boxed warnings, and warnings or precautions in the context of a specific patient, weighing risk versus benefit for that individual.
  • Differentiate between "not contraindicated" and "appropriate for this patient, " and identify where in the monograph such judgments are supported.
  • Reconcile Special Alerts with monograph content when they conflict, recognizing that alerts often take precedence until reconciled.
  • Apply renal and hepatic dose adjustment logic using current patient data, including labs and weight, rather than simple numeric substitution.
  • Identify missed interactions, stale patient data such as outdated labs or weights leading to incorrect dosing, and monograph guidance superseded by newer Special Alerts.
  • Judge whether the full clinical picture has been considered, including all active medications, current labs, and comorbidities, or whether the clinical question was scoped too narrowly.
  • Distinguish between a drug that clears interaction or contraindication checks and a drug that is clinically appropriate for the patient, flagging poor choices even when allowed by checks.
  • Verify that correct clinical conclusions are carried through into orders, prescriber notification, chart documentation, and care-team communication.
  • Support patient education tasks, including drug identification and preparation of multi-language handouts when relevant.
Qualifications
  • Regular use of UpToDate Lexidrug or Lexicomp in current or recent role, at least weekly.
  • Minimum 2 years of professional pharmacy experience, inpatient or hospital experience strongly preferred.
  • PharmD with an active pharmacy license, or equivalent verification-heavy clinical experience.
  • Fluent, reflexive use of Interactions, Trissel''s IV Compatibility, and monograph sections, able to spot missed interactions, stale dose calculations, or superseded guidance on sight.
  • Strong working knowledge in at least 2 to 3 of the following areas, demonstrated in practice: Trissel''s IV Compatibility, pediatric weight-based dosing, Beers Criteria and geriatric precautions, pregnancy and lactation risk, REMS and prescribing restrictions, therapeutic class comparisons, or drug identification and patient education.
  • Comfort evaluating whether a correct clinical finding was properly implemented in orders, charting, and communication with the care team.
  • Prior experience with data annotation, labeling, or clinical AI evaluation is a plus.

Compensation
  • Pay range: $75 to $120 per hour.
Eligibility
  • Must use UpToDate Lexidrug or Lexicomp regularly, weekly or more, as part of current or recent professional duties.
  • At least 2 years of professional pharmacy experience is required.
  • PharmD with active pharmacy license is preferred, or equivalent clinical experience that is verifiable.
